How we work
We work alongside the person and relevant partners to agree clear outcomes. Support is intended to strengthen skills, confidence and participation while ensuring that identified risks are managed proportionately.
Before support begins, we discuss the person’s needs, strengths, preferences, communication style, routines, risks and desired outcomes. The agreed support plan is reviewed so that care remains appropriate and responsive.
