Working together
We work with individuals, relatives, advocates, social workers, commissioners, occupational therapists, health professionals, discharge teams and housing partners. We value clear communication and shared planning while respecting consent, confidentiality and professional responsibilities.
We can consider referrals for
Information that helps us respond
- Name, age and current location
- Reason for referral and desired outcomes
- Current support arrangements
- Relevant health, mobility and communication needs
- Known risks and safeguarding information
- Personal care and medication requirements
- Preferred start date, days and times
- Funding route and commissioning contact
- Relevant assessments or reports that may lawfully be shared
Our response to referrals
We review available information, clarify gaps and assess whether we have the capacity and competence to meet the person’s needs. A package is not confirmed until assessment, risks, staffing and funding arrangements are sufficiently clear.
