Support shaped around the individual
Our community outreach service supports adults who need help to stay active, connected and confident outside the home. It can be arranged as a standalone service or alongside home care or supported living.
This service may be helpful for

How we work
Outreach support is practical and encouraging. We work at the person’s pace, helping them make choices, build confidence and do more for themselves while remaining safe.
Before support begins, we discuss the person’s needs, strengths, preferences, communication style, routines, risks and desired outcomes. The agreed support plan is reviewed so that care remains appropriate and responsive.
