Reablement is short-term, goal-focused support for adults who need help to regain confidence and everyday skills after illness, a fall, hospital admission, a change in mobility or a period of reduced independence.

How we work

The emphasis is not simply on doing tasks for the person. We agree practical goals and encourage safe participation so the individual can recover skills and confidence wherever possible. Any therapy or clinical recommendations must come from the relevant qualified professional.

Before support begins, we discuss the person’s needs, strengths, preferences, communication style, routines, risks and desired outcomes. The agreed support plan is reviewed so that care remains appropriate and responsive.
